At Home Dyslexia Screening Agreement

I understand that the ‘At Home Dyslexia Screening’ provided by LEARN READING will tell me whether or not my student fits the dyslexia profile, but will not qualify my child, or determine eligibility, for Special Education services or a 504 Plan in the public school system. 

I understand that if directions are adhered to, the results from this screening will determine with a high degree of accuracy whether my child fits the classic definition of dyslexia and what types of informal accommodations, next steps, and resources are recommended and available for students with dyslexia.

I understand that April McMurtrey is not a psychologist, will not be conducting an intelligence test or the tests required to determine eligibility for special education or to receive a formal diagnosis of “Reading Disorder” or “Learning Disability”.

I understand that April McMurtrey is not in the medical field, that dyslexia is not considered a medical condition, and that April McMurtrey does not accept medical insurance as payment for testing.
